Camper Registration Instructions

Dear Camper and Parents/Guardians,

Hello from Camp BASIC.  Camp BASIC 2013 looks like it will be an exciting week – we hope you can join us for the fun and fellowship. 

The dates for Camp BASIC 2013 are:

Week I, MONDAY June 10th - SATURDAY  June 15th
Week II, SUNDAY June 16th - FRIDAY June 21st
You have your choice of which week you would like, on a first come, first serve basis, so get your camper registered early!  Returning Camper Registration Dates are from January 15, 2013 - March 15, 2013.  New Camper Registration Dates are from March 16, 2013 - April 15, 2013.  If you wish to use the paper registration forms that were mailed to you or that you downloaded from our site, returning campers please have your Camper Registration Packet returned to Camp BASIC by March 15, 2013.  New Campers please have your  Registration Packet returned to Camp BASIC by April 15, 2013.

 

Please keep in mind that Camp BASIC is staffed with volunteers and designed for persons with a moderate developmental disability, and who are 10 years of age and older.  The facility we use is also designed for persons with a moderate physical disability.  Please consider that camp activities require some strength and stamina.  Campers need to participate in activities.  Campers should be able to walk independently and have independent toileting and feeding skills.  Our camp counselors are volunteers, primarily teenagers, who may not be experienced in giving personal care such as when a camper has a toileting accident on a regular basis.  Our camp also has no laundry facilities.  Campers that require wheelchairs or assistance with toileting may apply, but will be accepted dependent on available volunteers to help with those needs.  If a camper is requiring more assistance at camp than indicated on the application, the care giver will be notified and may be asked to pick up the camper.  Camp BASIC reserves the right not to accept any campers if the camp staff feels they will not be able to satisfactorily meet the campers needs. 

2013 Camp BASIC Fee is set at $300. 

Due to the rising costs of the State Park Lodging, increased food cost, and our desire to try to have a nurse on staff for each week, the Camp BASIC Board of Directors had no choice but to raise the fee from $200 to $300 for the week.  We researched many similar camps in Wisconsin and surrounding states and learned that we are still far below what other campers pay ($450 - $650 for a week).  Camp BASIC is non-profit.  We have no paid staff.  We simply try to break even every year.  Any extra money is used for promotional materials and a new video which we will use for recruiting new counselors.  We hope this increase will not inconvenience our campers.  Campers can still apply for camperships if there is a financial need. 

Everyone is responsible for a $100.00 deposit fee.  The balance of the fee is due on the first day of Camp, unless there has been a request for a scholarship.  Scholarship funds are available for the remaining $200.00 for those who may need them.  A scholarship request must accompany the paper registration form.  There is a place on the online form to make your request. 

If you register online, you must be ready to make the $100 deposit using your credit card, debit card, or your PayPal account.  Once you complete filling out the registration form, you will be directed to the PayPal web site to make that deposit.  You do not need to have a PayPal account set up in order to use PayPal.  They will allow you to use a credit/debit card without registering.  It is safe and secure, so you do not need to worry about someone getting your information.  All online camper registrations need to pay online in order to register.  If you do not wish to pay online, just print off a paper copy of the registration form and mail it to us with your check.

Every blank on our paper and online registration forms must be filled in.  If the information is not applicable to the camper, write N/A in the space provided.  We would encourage you to attach any extra information you feel may assist us in providing the best care possible.  You can e-mail this information to the appropriate director.  Their addresses are listed on the bottom of this page.  Sending a photograph of your camper is encouraged, but optional.  CAMPERS MUST HAVE COMPLETED FORMS IN ORDER TO ATTEND CAMP.  No exceptions will be made.

 

Medical and Medication Information

Please download our Medication Information Form.  Fill it out and bring it with you to camp.  The Medication Information Form must be filled out completely.  Please be aware that Camp BASIC does not have trained medical professionals on staff.  In the absence of a licensed nurse, Camp Directors will pass out medications and PRNs.  For this reason it is very important that all medical information is complete and instructions for any medications are clear.  If a significant medication change, health condition or surgery occurs in the month prior to Camp, please call the Director prior to Camp to discuss how the change may impact the camper’s experience at Camp.

Medications should be brought to camp in properly labeled packages to speed along the process of checking them in.  Bubble packs are preferred, but you may also put meds in small, brown envelopes as long as the camper’s name, the name(s) of the med(s), the dose(s), and when the med is taken is clearly indicated.  We do not have room to check-in meds in their original bottles – please do not bring them.  Be advised that Camp staff cannot give out unidentified medication.

If the camper requires menstrual pads, Depends or if the camper is diabetic or for any other reason needs between meal snacks (campers receive 3 generous meals a day), please provide an adequate supply, labeled with their name, for the time they will be at Camp.

Please note:  Camp BASIC does not supply wheelchairs.

In an Emergency

In the event of medical emergencies or an illness, the Camper will be taken to Prairie du Chien Memorial Hospital.  Some members of Camp staff will be certified in Basic First Aid and CPR.

Campers will be notified on their status of acceptance to Camp BASIC before May 15, 2013.
